The Personal Directive Agent Workbook has been designed to support you (the Estate Planner) and your Personal Directive Agent in planning for future incapacity.   The Personal Directive (a legal document) comes into effect when the Estate Planner is no longer able to take care of his or her own affairs.

Assigning a Personal Directive Agent is a legal activity in which you (the Estate Planner) appoint someone to act on your behalf should you become mentally or physically incapacitated. 

The Personal Directive form outlines which activities can be undertaken on your behalf, however, this document does not cover all of the detail the Personal Directive Agent will need to ensure your wishes are carried out appropriately and your well-being is attended to.   

To be considered legal, these documents must be created signed and witnessed while you are still capable of making the decisions about whom you wish to name as your representative and the specific content that is itemized in the documents. 

This Workbook will walk you through a series of considerations that will help ensure you and your appointed representative  establish a mutual understanding.
